About the role

This role is an exciting opportunity to make a real difference in people’s lives by supporting adults to live independently and achieve their wellbeing goals. As a Support Planner, you’ll work closely with individuals, families and professionals to assess needs, shape personalised support plans and promote strengthbased, communityfocused solutions. You’ll help people build independence, explore employment opportunities and access the right support at the right time. With varied work, meaningful impact and the chance to empower others every day, this role is perfect for someone who’s passionate about personcentred care and positive change.

The post holder is expected to cover across 7 days a week, as part of a rota.


The post holder will be expected to undertake a certain amount of travelling in the course of his/her duties. Appropriate mileage rates will be payable for car users.
