Blackpool Council is looking for an enthusiastic Transfer of Care Co-ordinator to play a pivotal role within our Home Care and Reablement Service. This is an exciting opportunity to be at the heart of service delivery, helping people transition safely from hospital to home while ensuring our care teams are deployed efficiently to meet growing demand.

Every day, you'll work alongside health professionals, social care teams, care providers and families to coordinate support that enables people to remain independent and thrive within their communities.

About the Role

As Transfer of Care Co-ordinator, you will be a key operational link between health and social care services, ensuring referrals are processed effectively and care provision is coordinated seamlessly.

You'll:

-          Coordinate transfers of care from hospital and community settings

-          Lead daily service huddles to identify priorities and manage demand

-          Allocate and deploy Home Care staff through electronic rota systems

-          Work closely with the Transfer of Care Hub and multidisciplinary partners

-          Monitor service capacity and identify solutions to meet demand

-          Support service performance reporting and funding-related data requirements

-          Provide coaching and operational support to administration colleagues

-          Liaise with service users, families, care providers and professionals

-          Help ensure every service user receives timely, high-quality support

About You

We're looking for someone who thrives in a fast-paced environment and enjoys balancing priorities while maintaining excellent attention to detail.

You'll bring:

-          Experience working within health, social care or a related care environment

-          Strong organisational and coordination skills

-          Excellent communication and relationship-building abilities

-          Confidence using ICT systems and electronic scheduling tools

-          The ability to analyse information and make informed decisions

-          A proactive and solution-focused approach

-          Experience working with multidisciplinary teams and partner organisations

A Level 2 qualification in Health & Social Care (or equivalent), or a willingness to achieve this, is essential.
