Contract: Permanent 30hrs per week
Role Specifics: Must be over 18yrs of age and hold QCF (NVQ) 3 in Care or have a willingness to work towards this.

The Role
The Hub Coordinator supports highquality home support by supervising Resource Planners and ensuring effective work allocation. The role includes screening referrals, prioritising needs and ensuring timely responses.

You will monitor performance, maintain accurate scheduling and audit allocations for compliance with national standards. Working with colleagues, partners and carers, you will contribute to care planning, risk management and the promotion of independence and wellbeing at home.

The role also involves fostering strong team practice and providing clear leadership. Flexibility in working hours is essential to meet service needs.

About You
We’re looking for confident, proactive coordinators who thrive in a fastpaced environment. You’ll lead and support our Home Support Team, ensuring individuals receive safe, highquality care at home.

Strong leadership skills are essential, as you’ll guide, motivate and support staff to deliver their best. If you’re organised, calm under pressure and great with digital systems, you’ll fit right in. You’ll bring people together, solve problems and help create positive outcomes every day.

You must hold a Level 3 qualification or equivalent or be willing to work towards achieving the qualification.
